Why The Transition Project?
- Transition means 'change', and during our lives we will all go through a number of changes;
- Becoming a adult is a big step for people from 13 to 25 years old;
- All young people have a right to make decisions and choices about their future;
- The Transition Project provides information, advice and guidance;
- The Transition Project identifies what support young people need.
Full Potential
The Transition Project will provide you with a week or weekend focusing on your life and social skills, during which you will participate in lots of different activities and meet many new friends.
At the end of the week/weekend, we will help you develope your Individual Learning Plan (ILP) which will help you to achieve your dream goal, employment, college or university courses, and youth and leisure opportunities.
We will be providing different courses throughout the year. e.g. Driving Theory Test practices, computer courses, etc., which will help to prepare you for the future.
